St. Catherine of Siena Nursing & Rehabilitation Care Center has been serving Smithtown and its surrounding communities since 2000. It is a 240- bed, hospital-based, skilled nursing facility providing sub-acute, short-term rehabilitation and long-term care, as well as specialized dementia care programs. St. Catherine of Siena provides quality, resident-centered care in an environment that offers all the comforts of home, including a lovely garden, as well as amenities such as onsite banking, an onsite salon and an auditorium.
The expanded rehabilitation department is open 7 days a week and is staffed with 25 licensed, professional specialists in physical, occupational and speech-language pathology. The staff offers quality, state-of-the-art rehabilitation programs and modalities, to ensure patients’ transition into their homes and communities at their maximum level of physical functioning. In addition, specialized programs in cardiopulmonary, stroke rehabilitation, wound care, pain management, amputee and orthopedics are offered, to better assist patients with a wide variety of health management challenges. St. Catherine of Siena Nursing & Rehabilitation Care Center was awarded five stars by the Centers for Medicare & Medicaid Services and again named one of the Best Nursing Homes by U.S. News & World Report (2014–2015).
Opened in November 1991 under the auspices of Episcopal Health Services as Bishop Jonathan G. Sherman Episcopal Nursing Home providing long term care to the geriatric population including those affected by Alzheimer’s and dementia and those requiring sub acute care ,we began providing short term rehabilitation care in 1998 on a small scale- about 10 beds. -. In March of 2000 we were purchased by Catholic Health Services and re-named St. Catherine of Siena Nursing Home. We have grown to 100 beds of rehab and 40 beds for dementia care and 100 beds of long term care. We opened our new state of the art rehab gym in March of 2010.
St. Catherine of Siena Nursing and Rehabilitation Center is a long-term care facility of choice in northwestern Suffolk County, and is well known in the health care community throughout Suffolk and Nassau for its excellence and for the dedication of its staff and volunteers. Services include:
