St. Catherine of Siena Nursing & Rehabilitation (Smithtown, NY) has a dedicated care team who helps patients and residents, along with their family and caregivers, acclimate to short-term and long-term care.

The vast resources of St. Catherine of Siena Hospital ensure that the best medical care is offered to all residents. All emergent medical care needs, testing and physicians are immediately available to our entire resident population through the convenient tunnel system, including coordination of the delivery of dialysis services and wound care services.

Social Services

Social workers help residents and their family members adjust to full-time living at St. Catherine of Siena Nursing & Rehabilitation. They maintain an ongoing relationship with both residents and their families to offer counseling, guidance and support. Social workers are part of an interdisciplinary team that ensures residents' rights and needs are addressed on all levels.

Support groups, such as the Residents' Council and Family Council, provide an opportunity for the sharing of ideas as well as interaction with each other. Social workers provide education regarding advanced directives and benefits for veterans.

Social workers also coordinate a discharge plan for patients who are returning to the community after attending our rehabilitation program. This plan involves working with community agencies, home care agencies, insurance companies, and medical equipment providers to seamlessly facilitate a patient's safe return to home. Social workers are able to guide patients and their family members through the challenging decision-making process that arises when a patient is ready for discharge.

Admissions

To be eligible for admission to St. Catherine of Siena Nursing & Rehabilitation, an individual must require 24-hour skilled nursing care, rehabilitation/sub-acute treatment and /or hospice/palliative care. Our admissions staff will answer any questions you may have about eligibility for admission. In addition, our admission's department offers tours of our facility.

Prior to an applicant's consideration for admission, a New York State assessment form (PRI and Screen) must be completed by a PRI registered nurse. These forms are reviewed by our admissions team to determine eligibility for care. Our finance staff then helps determine the applicant's payment source(s). Upon availability of an appropriate bed, the eligible patient is admitted.

Medicare and Medicare/managed Care Insurance cover an eligible patient's stay for a defined period of time. Medicaid covers the cost of long-term care for those who qualify. Patients, who require stays not covered by insurance, may pay privately.
